I, the undersigned, wish to voluntarily serve as a volunteer for F.a.b. - Flourishing after ball, (hereinafter referred to as f.a.b.). I understand and acknowledge that volunteering with f.a.b. may involve inherent risks, and I hereby agree to the following terms and conditions:
Assumption of Risk: I understand that volunteering may involve physical, emotional, and other risks. I willingly assume these risks, both known and unknown, and acknowledge that my participation is purely voluntary.
Release of Liability: In consideration for being allowed to volunteer for f.a.b., I hereby release and discharge the Organization, its officers, directors, employees, and agents from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my volunteer activities. This includes any claims for personal injury, illness, or damage to property.
Medical Treatment: I understand that in case of an injury or illness that requires medical treatment during my volunteer work, f.a.b. is not responsible for providing such treatment or covering any associated costs.
Confidentiality: I agree to maintain the confidentiality of any sensitive information I may have access to during my volunteer work, including but not limited to personal information of individuals served by f.a.b.
Photographic Release: I grant permission to the Organization to use photographs, video
